Food Service Permits

Forms, requirements, and instructions for restaurants, mobile food units, temporary food events, and other food service activities

Temporary Food Establishments

Temporary food establishment” means a food establishment that operates for a period of time not to exceed 21 days in one location, affiliated with and endorsed by a transitory fair, carnival, circus, festival, or public exhibition. Food establishments that operate in the same event location for more than 21 days per calendar year are not eligible for a temporary food establishment permit. Domestic yard sales and businesses such as auctions, flea markets, or farmers markets are not eligible for a temporary food establishment permit.

To operate a Temporary Food Establishment (TFE), both the TFE Coordinator Application and individual TFE Vendor Applications must be submitted in full at least 15 days prior to the event.

Required Details:

TFE Coordinator Application 

must include a complete list of participating vendors.

Individual TFE Vendor Applications

  • Each vendor must submit an application

  • $75 fee required per vendor

Important Notes

  • The Health Department will only accept vendor applications approved by the designated TFE Coordinator.
  • Applications submitted after the 15-day deadline will not be considered

  • Vendor applications submitted without coordinator approval will not be accepted

Mobile Food Unit or Push Cart Applications

Required Materials:

  • Completed Application Form

  • Proposed Menu

  • Commissary Agreement Form (fully completed and signed)

  • Equipment Specification Sheets for all appliances or food service equipment

A GVPH staff member will contact you to collect payment by credit card.

 

Mail Submission

Mail the completed packet with a check to the Environmental Health office in the county where your commissary is located.

 

Important Note

Incomplete packets will delay processing. Please review the submission checklist before sending your application.

Food Service Establishment Plan Review Submission Requirements

 

To submit a complete Plan Review Application for a Food Service Establishment, please ensure the following items are included:

 

  1. Completed Plan Review Application Form
  2. Proposed Menu
  3. Equipment Specification Sheets
  4. Site Plans or Building Plans (required for new facilities)

 

Note: If the application is for an existing facility undergoing a change of ownership, building plans may not be required. Please confirm with Environmental Health before submitting.

 

Incomplete applications may result in processing delays. Be sure to include all required materials before submission.

Public Pools

Permit forms, operational guidance, and supporting resources for public swimming pools.

Application materials may include:

Public pool operators are encouraged to submit application materials as early as possible, especially before peak seasonal permit activity. Incomplete submissions may delay review and inspection.
